Employee Injury Form & Cover Page

In the event an employee is injured at work please complete the Employee Incident/Injury Form.  The SAU would appreciate it if staff would also complete the Voluntary Cover Page.  Both forms should be submitted to the SAU and a copy to the building nurse or principal within 24 hours of the incident/injury.  If you have any questions please contact your building principal.
